Super Bill Provided
For submission to your insurer
We are out-of-network providers and do not bill insurance plans, Medicare/Medicaid, or Tricare directly. However, clients with out-of-network benefits may be eligible for reimbursement, depending on their insurance plan.
We recommend contacting your insurance provider before scheduling an appointment to understand their out-of-network mental health service policies, including coverage percentages, claim submission procedures, and
applicable deductibles.
Reimbursement decisions are made by your insurance provider. We will provide a Super Bill after your sessions for you to submit to your insurer, but we cannot control the reimbursement process or outcomes. [Please note that educational services are not considered healthcare and therefore not reimbursable by insurance.]

Disclaimer:
As of January 1, 2022, the No Surprises Act was enacted to protect clients from unexpected medical and mental health care costs. As a Licensed Clinical
Social Worker (LCSW) in private practice, I believe in transparency, ensuring that you have a clear understanding of the costs of therapy before beginning
services.
Your Rights Under the No Surprises Act
- Right to a Good Faith Estimate (GFE): If you are uninsured or choosing not to use insurance for services, you are entitled to a Good Faith Estimate of the expected costs for therapy.
- Clear and Predictable Fees: You will receive an estimate of the cost of services in advance. While actual costs may vary depending on the length and frequency of treatment, I will work with you to ensure affordability and transparency.
- No Surprise Billing: If you receive a bill for significantly more than your Good Faith Estimate, you have the right to dispute the charges through the U.S. Department of Health & Human Services (HHS).
What to Expect from a Good Faith Estimate
Before starting therapy, you will receive a written Good Faith Estimate that outlines:
- The cost per session (e.g., individual therapy, family therapy) $200 for Elizabeth Wilkins-McKee, LCSW and Laura Kelloway, LCSW
- The anticipated number of sessions based on your needs, typically minimum of 8 3 12, reevaluated during each session
- Any additional costs for services (e.g., assessments, consultations)
- A timeframe for reevaluating treatment plans and costs. Treatment planning is done on an ongoing basis during each session. Should there be a change in your rate you will be notified.
This estimate does not obligate you to a specific number of sessions, and therapy can be adjusted based on your evolving needs.
What This Means for You
- You will not receive unexpected or excessive bills.
- You do not have to commit to all estimated sessions upfront.
- You have the right to a revised estimate if your treatment plan changes.
- If your actual costs exceed your estimate by more than $400, you have the right to dispute the charges.
